At Families and Connections Collaborative Services, our mission is to provide compassionate, person-centered care that supports individuals and families in living safe, independent, and fulfilling lives. We are committed to promoting dignity, inclusion, and well-being through high-quality home and community-based services tailored to meet each person's unique needs.

In-home support from licensed professionals, CNA's and Home Health aides to manage chronic conditions and recovery needs.
Medication management, Personal care, Companion, Chore, PAS, mobility assistance, and health monitoring.
🔹 Developmental Disabilities Support
Certified services for individuals with intellectual and developmental disabilities.
Habilitation, in-home support, community engagement, Res and Day supports, Child Day habilitation, Independent Living, Homemaker, Shared Living Provider, Family Support, Respite, Transportation, Life skills coaching and a wide range of other services.
🔹 Post-Operative Care
Safe, in-home recovery care following surgery or hospital discharge.
Wound care, pain management, daily living support, and follow-up coordination
🔹 TBI (Traumatic Brain Injury) Care
Compassionate, structured care for individuals living with TBI.
Supervision, safety support, cognitive routines, and family guidance.
🔹 Juvenile Probation Support Services
Structured support for youth involved in the juvenile justice system, aimed at reducing recidivism and building life skills.
Services include mentoring, behavior coaching, community service coordination, family engagement, and reentry support.

Meet the Founder & CEO
Janelle Hardiman, CEO & Founder
Licensed Nurse | Advocate | Entrepreneur
I’m Janelle Hardiman, the Founder and CEO of Families and Connections Collaborative Services, LLC. I am also a licensed nurse with over two decades of experience in healthcare and human services. My career began at the frontlines—providing direct care and support to individuals with intellectual and developmental disabilities. Over time, I stepped into leadership roles where I saw the critical need for compassionate, personalized, and culturally responsive care.
I founded Families and Connections Collaborative Services to create something different—a care model built on relationships, advocacy, and true community impact. As a Black woman, a nurse, and a dedicated caregiver, I understand the unique challenges faced by both individuals and families navigating support systems. My goal was to build an agency that listens, understands, and delivers real solutions with heart.
Today, I lead a team of skilled, passionate professionals who are committed to improving lives through high-quality home and community-based services. Whether it’s home health, developmental disabilities support, or post-operative care, our mission is rooted in dignity, empowerment, and trust.
